Rust’s three core medical items are not an upgrade ladder. Bandages are cheap bleed control, Medical Syringes protect the health you need for the next exchange, and Large Medkits trade immediate recovery for a deep heal and a special belt-slot recovery guarantee. The expensive item is therefore not automatically the right panic button: choose the job first, then the item.

The Medical Syringe Is the Best Panic Heal

The table’s key distinction is timing: the Medical Syringe supplies the most health immediately, while the Large Medkit puts most of its value into delayed healing. The Medkit also stacks to one, so carrying more than one consumes a new slot each time.

Bandages Control Bleeding; Syringes Restore Fighting Health

When another hit may be coming, choose the Medical Syringe for health: its 15 instant health is the largest immediate return in the comparison. Use a Bandage when bleeding is the problem and spending fuel or metal would be wasteful. Syringes rebuild your fighting margin; Bandages keep a manageable bleed from becoming the next problem.

Both items can also help a teammate. On PC, left-click heals yourself and right-click heals a target, so the same supplies can stabilize whichever player is holding the group back.

Avoid a universal stack count: a short solo recycle run and a long team push do not spend medicine at the same rate. Set the roles instead. Keep Bandages available if bleeding is likely, then devote the rest of your medical space to syringe stacks in proportion to the time and fights between resupply points.

A Large Medkit Is Wounded Insurance, Not a Bigger Syringe

A Large Medkit deserves a belt slot only when all three conditions are true:

  • You can spare a belt slot after your weapon, tools, and active combat medicine.
  • Being downed—not falling or being finished immediately—is a plausible way the route fails.
  • Losing the carried gear or objective costs more than replacing the medkit’s raw inputs.

Its distinct value appears when it occupies a belt slot and you enter the wounded state. If the normal recovery check fails, the medkit is consumed and you recover when the wounded timer ends. If that check would have succeeded normally, the medkit is not consumed. The guarantee requires the belt slot; keeping that slot available for another weapon, tool, or medical item means giving up the insurance.

An enemy can still finish you before the timer resolves, and Facepunch’s wounding notes exclude wounded states caused by fall damage. If either is the route’s likely failure, another syringe is the better use of the slot.

Craft Syringes Before Converting a Large Medkit

The crafting gates reinforce that order. A Bandage is known by default, needs no workbench, and costs 4 Cloth with a five-second craft. A Medical Syringe requires Workbench Level 2 and costs 15 Cloth, 10 Low Grade Fuel, and 10 Metal Fragments; it takes 10 seconds to craft there. The final Large Medkit craft consumes two syringes plus 10 more Low Grade Fuel and takes 15 seconds at Workbench Level 2.

From raw materials, that one medkit represents 30 Cloth, 30 Low Grade Fuel, and 20 Metal Fragments. The conversion turns two syringes’ combined 30 instant health and up to 40 delayed health into 10 instant and up to 100 delayed, while adding complete bleeding removal and the belt-slot guarantee. It does not save space at exactly two syringes: two fill one syringe stack, and one medkit fills one slot.

Build the syringe supply needed to keep fighting first. Convert the next two only after the three medkit conditions are satisfied.
